• Request corrections to your information

Additional protections may apply to substance use treatment records under federal law.

We are required to:

• Maintain the privacy of your information
• Provide this notice
• Notify you in the event of a breach when required

If you believe your privacy rights have been violated, you may contact our office or file a complaint with the U.S. Department of Health and Human Services.

Your care will not be affected by filing a complaint.

In addition to protections under the Health Insurance Portability and Accountability Act, certain records related to substance use treatment are protected under federal law, including 42 CFR Part 2, which provides stricter confidentiality requirements.

These regulations limit how information related to substance use treatment may be disclosed without your written consent, except in specific circumstances permitted by law.
